Exercisabilities is Preventing Type 2 Diabetes with a proven program called the DPP (Diabetes Prevention Program).  Exercisabilities is the only site in Southeast MN to get the CDC recognition as an official delivery site and the first in the state to be approved as a Medicare Diabetes Prevention Program.  That means that if you qualify (see below) you can likely participate in the full year program under full insurance coverage.  Insurances that are covering the program at EA include:

  • Medicare
  • MN Medicaid
  • Ucare
  • BCBS
  • United Health Care
  • Mayo Clinic Health Solutions
  • And more!

To qualify, you must:

  • 18 years or older
  • No previous diagnosis of diabetes
  • BMI > 24 (22 if asian) (example 5’8” and greater than 170 lbs)
    At least one of the following in the past year (need a copy of the Lab report on file)\

    • HbA1C: 5.7% – 6.4%
    • Fasting plasma glucose 100-125 mg/dL (Medicare requires 110-125 mg/dl)
    • 2 hour plasma glucose (after a 75 gm glucose load): 140-199 mg/dL
    • Previous diagnosis of gestational diabetes (is not a qualification for Medicare)

OR

you can TAKE THE TEST  to see if you qualify. However, your insurance will likely only cover the program if you have the above items.  You can participate for just $750 for the full year even if your insurance does not pay or if you do not have the above qualifications as a weight loss program.

The Program:

26 sessions over 1 year. This is a research program. Regular attendance is required as well as a goal attainment of 5% weight loss in the first 6 months.
Free access to exercise equipment through gym pass 2 days per week from 5-6 for the first 6 months. Learn to be fit class 5-6 PM each Monday and Wednesday staffed with an exercise specialist for circuit strength and cardio training instruction.

Lose up to 7% of your current weight while learning to how to change your lifestyle to support better eating, become more active, and get improved health reports!

Join ExercisABILITIES biggest fundraiser of the year on August 25th, 2018!  This adventure filled, family friendly event is fully accessible for all ages and abilities to join the fun.  Ironwood Springs Christian Ranch offers many opportunities for the whole community to explore nature and live a bit on the wild side while getting a little exercise and maybe even trying something new!    All proceeds go to support our many programs and services at EA.

ExercisAbilities Receives Award from Otto Bremer Trust

$30,000 for general operations to provide rehabilitation services to individuals with physical, cognitive, and medical unstable conditions

 ExercisAbilities Awarded Otto Bremer Grant

ROCHESTER, Minn. – ExercisAbilities was recently awarded a $30,000 grant from the Otto Bremer Trust. The grant will be used toward support of our services and programs to support rehabilitation services to under-served individuals with physical, cognitive, or medically unstable conditions at ExercisAbilities.

“This grant will support our operations to improve our ability to provide rehabilitation services to many in our community who do not have insurance money to support their ongoing recovery needs” explains Melanie Brennan, ExercisAbilities Director and Founder. “Our current health care environment provides very little financial assistance to those with disability to achieve even the most basic physical activity or rehabilitation that is very much needed to maintain an adequate quality of life, let alone make any improvements for them to better participate in life at home, work and the community.”

“The money will help us support operations and staffing to extend our Forever Strong Exercise Therapy and Independent Living Skills to reach more individuals in Southeast Minnesota.” adds Brennan.  “We are so thankful for the Otto Bremer Trust to move this program forward to fulfill an immediate need.”

$10,000 grant to develop a Pediatric Rehabilitation Outpatient Program

 

ROCHESTER, Minn. – Exercisabilities was recently awarded a $10,000 grant from the Rochester Area Foundation. The grant will be used to develop a pediatric rehabilitation outpatient program at Exercisabilities.

“This grant is a blessing to the families in Rochester who are seeking access to high quality pediatric rehabilitation services” explains Melanie Brennan, Exercisabilities Director and Founder. “There is a current shortage in openings at area rehabilitation facilities for occupational therapy, speech therapy, and physical therapy, especially for children on the autism spectrum.”

 

“The money will help us purchase equipment that is unique and necessary to expand our services to children,” adds Brennan.  “We are so thankful for the Rochester Area Foundation to move this program forward to fulfill an immediate need.”
